Pre-Visit Requirements.

No deposit required to apply
Must be 18 years of age or older to apply individually.
For minors, less than 18 years of age, a caregiver is required at the time of the visit.
Proof of Identity.
No deposit required. The full price to be collected at the time of the visit is $150 or $125 for disconted applications. This includes the first evaluation with a full consultation from the doctor and review of records. Free follow ups are offered during the next 60 days after the first evaluation
In the circumstance that the initial screening finds you ineligible, you will be reimbursed the full amount back. No fees charged.
In case we find you eligible and certify you, but the state rejects your application, will refund you the full amount back. Risk free evaluation. Proof of state final rejection is required for refund.
The Compassionate Discount Price is extended to Military Veterans and minors. Other categories may apply on case by case bases, to be discussed at the time of the visit.
Optional only: Please bring your any available previous medical records. In case medical records are not readily available, then bring your previous prescriptions/ Pharmacy records / Xrays/ MRIs, and any supportive previous records to confirm the qualifying medical conditions. If you do not have any, will be able to access your records at the major oklahoma healthcare providers at the time of the visit.
All paperwork is available at the clinic and can be filled at the time of the consultation, but if you wish to expedite your visit, you can print and fill in the forms and bring them with you to your visit.
